Gastrostomy Tube care Feeding * Feeding may be given in the form of bolus feeds or drip feeding as directed by your doctor. * The amount and type of feeds given has to be increased according to the age and weight of your child. Till the age of 6 months only milk shall suffice. After the age of 6 months other food articles in the form of cereals/ vegetables/ fruits may be added in the pureed and liquefied form, care being taken to see that the tube does not get blocked. * At the end of each feeding and after every medication, the tube must be flushed or rinsed with 5 to 10 ml of water to prevent the tube from becoming clogged with formula or medication. * All the prescribed medications need to be given in liquid form. * OPV ( oral polio vaccine) needs to be given through the gastrostomy. * In children with esophagostomy ( opening made in the neck for egress of saliva) , at the time of giving tube feeds, oral feeding ( sham feeding) should be done so that the child learns to associate the feeling of food in the mouth with the sensation of stomach getting filled and hunger satisfied. Baths Children can take a bath 4 days after the operation. Change the dressing after the bath if it is wet or loose and no longer stabilizes the tube. Tummy Time After gastrostomy surgery, avoid placing your child on his stomach for at least 2 weeks. It is okay if the child rolls unassisted onto his stomach and seems to tolerate the position. Your child may resume physical therapy one week after the operation but should not be placed on his stomach or on a ball during therapy for 3 weeks. Changing the tube Depending on the type of device your doctor shall schedule tube changes for your baby. Problem arises when the tube comes out accidentally or gets blocked. The opening in the skin for the tube closes within a few hours if the tube is not inserted into it. Eventually, a formal operation may be required to reinsert the tube. Thus, the best option is to rush to your doctor immediately to get the tube repositioned. A better alternative is to learn to insert the tube yourself for such situations when medical help is not nearby. The technique for insertion of a simple Foley’ s catheter is mentioned below: * Always keep a catheter of suitable size ( ask your doctor about the size suitable for your kid) , lubricating jelly and a 2cc syringe handy. * Take out the catheter from its wrapper. * Fill up the 2cc syringe with clean water up to 2cc mark. * Push the nozzle of the syringe into the 2nd hub of the catheter through which feeds are not given, fit it tightly ( it has got a spring mechanism) , and push in the fluid. You can see the balloon getting inflated with the water which should remain inflated when the syringe is removed. This tests that the balloon mechanism of the catheter is working well. Now, reinsert the syringe and take out the fluid – the balloon should deflate. Foley’ s catheter with deflated balloon Balloon inflation Foley’ s catheter with inflated balloon * Apply lubricating jelly liberally to the tip of catheter. * Insert the tip of the catheter into the opening in the skin till the time the length occupied by the balloon and an extra length of 2 – 3 cm are inserted ( ie. around 4-6 cm) . * Inflate the balloon again as above. Apply a gentle tug to the catheter to check whether ir is firmly secured.
